The HIV epidemic in the United States is currently dominated by drug resistant viruses. Most HIV drug resistant mutations (DRMs) affect reverse transcriptase, and primary mutations affect regions V and I, with secondary mutations affecting regions V, II, and VI. The prevalence of HIV DRMs varies among populations, and is highest in patients initiating combination therapy with nucleoside analogues and protease inhibitors. Resistance to all but a few of the currently available drugs has emerged during the past decade. Frequent genotypic resistance testing is therefore needed to monitor the prevalence of HIV DRMs and guide the selection of regimens. Recent studies have also suggested that the presence of HIV DRMs at the initiation of antiretroviral therapy can lead to rapid disease progression in some patients.Rational design and synthesis of a multifunctional lactam library for overcoming drug resistance.
Drug resistance remains one of the most devastating obstacles to successful chemotherapy. Herein, a rational design and synthesis of a multifunctional lactam library is described. A library of 99,600 compounds was prepared, and 5000 compounds were screened against S. aureus, M. tuberculosis, K. pneumonia, E. coli and H. pylori. Finally, a promising multifunctional compound was identified for potential application in anti-drug resistance therapy.
